Photo Release Form

Children under the age of 18 must have written verification that their guardian approves of their photos being posted. We aim to protect our children.

This waiver states: 

Denver United Methodist Church has my permission to use my or my child’s photograph for means of publicity including, but not limited to, print publications, online publications, presentations, websites, and social media.

I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
